I was one of the very intense beta testers as you'll discover when you finish the game.
 
Doors don't relock.
 
I suppose it is possible that if you unlocked the door but did not go through that could be an issue. I'm certain I didn't try that, but many of the other beta testers might have.
 
You have struck on the "key" -- use all the back ups, frequently but keep one in reserve. I used to back up each time a character leveled. Thus, if necessary I could restore not "too far" back. If you're linked to a pc, copy your backup files there frequently (to different folders each time) and you're set to restore from anywhere / anytime. I found playing with just two characters to be the most fun, but have completed the game with a single character several times. (This does require a number of frequent backups. If a single character is put down or paralyzed, it's over.) Also, strange things happen when you get more than three-digit magic or hit points. 
 
I did used to store "extra" interesting stuff on people that hang around in the inns.

Is it possible for the door to “re-lock itself?
 
What if I used the key, but never went through the door?  Then came back later and tried?  Could that be it?
 
Its really a mute point, I restarted my game over the weekend and have progressed past that point, I guess I should have kept the save game from that experience.

I seem to have "Lost" the saphire key in the cellar. I remember getting it, but noone in my party has it anymore and it is no longer at (5,4) where it is...
